Description

The Application/Questionnaire name is not listed in the dropdown menu while adding/creating a new Notification.

Cause

Enable Notifications is not checked under the Properties tab of that Application/Questionnaire.

Resolution

Check the Enable Notifications checkbox under the Properties tab of that Application/Questionnaire and Save it.
